Chinese Medicine Skin Disorders

1 Wind rash (Feng zhen)

It is similar as modern diagnosis of urticaria or hives, which is a kind of allergic skin disease with skin wheals.

Differentiations and treatments

1.1. Wind-heat

Clinical manifestation: severe pruritus, hot, red wheals, more severe with hot and better with cold, and red tongue with yellow fur  

Treatment principle: Dispersing the wind 

Herb treatment: Sanfeng Qingre Yin

1.2. Wind-cold

Clinical manifestation: severe pruritus, white wheals, more severe with cold and better with hot, and light red tongue with white fur  

Treatment principle: Dispersing wind-cold 

Herb treatment: Jufeng Sanhan Yin

1.3. Heat stagnation in stomach

Clinical manifestation: wheals after some foods, often with abdominal pain, red tongue with yellow fur  

Treatment principle: Clearing heat from stomach 

Herb treatment: Fangfeng Tongsheng San

1.4 Qi and Blood deficiency

Clinical manifestation: frequent wheals lasting several months even years, light color, worse after exertion, also tired, short breath, pale tongue with white fur  

Treatment principle: Tonifying qi and dispersing wind 

Herb treatment: Yupingfeng San

1.5 Un-harmonized Chong-Ren

Clinical manifestation: wheals frequent occur before periods and after periods the wheals disappear, slippery pulse 

Treatment principle: Harmonizing Chong-Ren

Herb treatment: Siwu Tang

2. Snake cinnabar (She chuan chuang)

It is similar as modern diagnosis of herpes zoster, manifested as groups of crowed blisters arranged in zonary fashion along one side of the nerve distribution.

2.1. Heat excess

Clinical manifestation: There are mild fever malice before some cases of skin lesion,  the lesion is groups of papule eruption with hot pain, red tongue with yellow fur  

Treatment principle: clearing heat from blood 

Herb treatment: Liangxue Qinggan Tang 

2.2. Damp-heat excess

Clinical manifestation: groups of blisters with hot pain, some rupture blisters with ooze, red tongue with yellow greasy fur  

Treatment principle: clearing heat-damp  

Herb treatment: Longdan Qinggan Tang

2.3. Blood stasis

Clinical manifestation: lesions gone but still skin purple and very painful, especially in seniors

Treatment principle: soothing liver, promoting blood circulation  

Herb treatment: Chaihu Shugan San and Shixiao San  

4.3 Eczema

It is allergic inflammatory, manifested as itching, polymorphic skin lesions and frequent recurrences,

Clinical manifestation: clusters of red papule, blister, erosions, exudeations and crust,  very itching, red tongue with yellow greasy fur 

Treatment principle: Clearing heat-damp

3. Spleen deficiency and damp retention

Clinical manifestation: blister, ooze (big and light), itching, big and pale tongue with white fur 

Treatment principle: Tonifying spleen and clearing damp 

Herb treatment: Jianpi Weiling Tang

4. Blood deficiency and dry-wind

Clinical manifestation: chronic, thick and rough skin with pigmentation, itching, pale tongue  

Treatment principle: Tonifying blood and clearing wind 

Herb treatment: Siwu Xiaofeng Yin

5. Damp-toxin

Clinical manifestation: pus, hot and pain blister, itching, red tongue with yellow fur 

Treatment principle: Clearing toxin-damp

4.4  Bai Bi

It is modern diagnosis of psoriasis, manifested as erythematous scaly lesion and itching.

1. Wind-heat

Clinical manifestation: serious itching, red papule, and scaly erythematous lesion with red halo around them, red tongue with yellow fur  

Treatment principle: Clearing heat and wind 

Herb treatment: Shuanghua Tufuling Yin

2. Blood-stasis

Clinical manifestation: itching, scales on erythema or the erythema with silver scales on the skin

Treatment principle: promoting qi and blood circulation to remove stasis 

Herb treatment: Huoxue Qufeng Tang

3. Blood deficiency

Clinical manifestation: mild itching, brown patches or pigmentation with fine furfures    

Treatment principle: Tonifying blood and clearing wind 

Herb treatment: Dangdui Yinzi

4.5 Cutaneous pruritus

It has no primary skin lesion but a sensation of serious paroxysmal itching on the skin.

Treatment principle: Tonifying blood and clearing wind

4.6 Patch balding (Ban tu)

It is sudden limited baldness.

Clinical manifestation: many alopecia area, yin-

Clinical manifestation: many alopecia area, blood-   

Treatment principle: Tonifying blood   

Herb treatment: Guipi Tang

3. Qi stagnation

Clinical manifestation: sudden alopecia, liver qi stagnation   

Treatment principle: soothing liver  

Herb treatment: Xiaoyao San

4.7  Erysipelas (Dandu)

It is a kind of acute inflammatory diseases caused by hemolytic streptococci that invades the reticular lymphatic vessels of the skin or the mucosa, which is marked  acute onset, chill and fever,  flaming reddish spots and pathches at the skin, very severe pain.

1. Wind-heat invading

Clinical manifestation: Red, pain  and swollen often in the face or head,  fever

Treatment principle: Clearing heat and wind

Herb treatment: Puji Xiaodu Yin

2. Heat stagnating in the liver  

Clinical manifestation: pain  and swollen often in chest, abdomen or hip,  fever

Treatment principle: Clearing heat from liver channel  

Herb treatment: Longdan Xiegan Tang

3. Heat-damp stagnating in the stomach

Clinical manifestation: often lower limb red, pain  and swollen,  fever

Treatment principle: Clearing heat and damp   

Herb treatment: Wushen Tang + Bixie Shenshi Tang

4. Heat-toxin 

Clinical manifestation: Severe red, pain  and swollen, high fever, delirium even coma 

Treatment principle: Clearing heat and toxin, and opening the mind 

Herb treatment: Qingwen Baidu Yin

4.8  Acne  Vulgaris

It  is a chronic inflammation of hair-follicle and sebaceous glands.  The skin lesion: first papulae in normal colour, gradually becoming red inflammatory papulae and nodules, some fatty squeezed out from big one, which is acne.  They can further develop deeper and bigger, also can be secondary infections.

1. Heat in the lung

Clinical manifestation: Red papulaes are main skin lesions

Treatment principle: Clear heat from lung

Herb treatment: Piba Qingfei Yin

2. Heat-damp in the stomach

Clinical manifestation: acne with fatty secretion and pus are  main skin lesions

Treatment principle: Clear heat-damp from stomach

Herb treatment: Yinchenhao Tang

3. Phlegm stagnation

Clinical manifestation: chronic acne, indurative acne, cystic acne, white tongue with white greasy fur 

Treatment principle: Tonifying spleen and removing phlegm 

Herb treatment: Sijunzi Tang

4.9  Rosacea

It is a chronic skin disease at the nose or around the nose.  The skin lesions can be divided into three stages: erythema (red nose), papulae and rhinophyma (nose becoming swollen and even larger with a rough and uneven surface raised up like small pustules.

1. Heat in the blood

Clinical manifestation: Red nose (erythema stage)

Treatment principle: clearing heat from blood

Herb treatment: Liangxue Siwu Tang

2. Heat-damp stagnation 

Clinical manifestation: Papulae stage,  possibly  with secondary infection 

Treatment principle: clearing heat and damp

Herb treatment: Longdan Xiegan Tang

3. Blood stasis 

Clinical manifestation: rhinophyma stage, swollen and purple nose

Treatment principle: promoting blood circulation and removing stasis

Herb treatment: Taohong Siwu Tang

4.10  Neurodermatitis

It is a kind of chronic skin disease with pruritic inflammation, often occurring in neck, elbow, knee and sacrum.  The skin lesion: first intermittent itching, then round clusters or polyangled pink papulae covered with scanty fine scales.  The skin becoming thick, dry and rough, even lichen.

1. Wind-heat

Clinical manifestation: New lesion, itch, reddish skin and crowds of papulae appear on it

2.  Blood-yin deficiency

Clinical manifestation: chronic lesion, itch,  lichen   

Treatment principle: tonifying blood and clearing wind

Herb treatment: Siwu Xiaofeng San
