Chinese Medicine Blood and Liquid Disorders  

This disorders may be caused by EPF invading and organs functions disorders and include liquid and blood metabolism or circulation disorders.

1. Retention of Phlegm and Fluid (Dan-Yin Syndrome)

It is a pathological changes caused by  excessive water in a certain part of the body, which results from the disorder of body transportation, transformation  and distribution.

1.1. Etiologies and pathogenesis

Invasion by cold-dampness

Improper diet

Deficiency of Yang-qi

1.2. Differentiation and treatments

1.2.1 Excess syndromes

(1) Retention in the intestines and stomach

Clinical manifestations: fullness and pain in the epigastria and abdomen, a splashing sound in the stomach or intestines, a deep, taut and forceful pulse 

Treatment principle: eliminating retained fluid with purgatives

Herb formula: Gan Sui Ban Xie Tang or Ji Jiao Li Huang Wan

(2) Retention in the chest and hypochondria

Clinical manifestations: distending pain in chest and hypochondria, aggravated by cough, movement of body and respiration, shortness of breath, palpitation, thin and white tongue fur, deep and taut pulse.

Treatment principle:  eliminating retained fluid with purgatives

Herb formula: Shi Zao Tang

(3) Retention in the chest and lung

Clinical manifestations: cough, dyspnea, fullness in the chest, orthopnea in severe cases, expectoration of profuse, white and frothy sputum, edema of the face and eyelids following long-standing cough, white tongue fur, a deep, taut  pulse 

Treatment principle: warming the lung and resolving phlegm

Herb formula: Xiao Qing Long Tang

1.2.2. Deficiency syndromes

(1) Spleen yang deficiency

Clinical manifestations: distended and full sensation in the chest and hypochondria, preference for warmth and pressure in the epigastric and abdominal region, a splashing sound in the stomach, back cold, nausea and vomiting, tongue fur white and slippery, a deep and weak pulse 

Treatment principle:  warming spleen to eliminate retained fluid

Herb formula: Ling Gui Zhu Gan Tang

(2) Kidney yang deficiency

Clinical manifestations: body cold, shortness of breath, short breath, palpitation, edema, tongue fur white and slippery, a deep and weak pulse 

Treatment principle:  tonifying kidney to eliminate retained fluid

Herb formula: Jin Gui Shen Qi Wan

2. Sweating Syndrome (Han Syndrome)

It is an abnormal sweating, mainly includes spontaneous and night sweating.

2.1 Etiologies and pathogenesis

Lung qi deficiency

Disharmony of Ying and Wei qi

Yin deficiency

Pathogenic heat

2.2 Differentiations and treatments

2.2.1. Lung qi deficiency

Clinical manifestations: spontaneous sweating, aversion wind, easily catching cold, pale tongue with white fur, thin and weak pulse.

Treatment principle: tonifying qi to consolidate body surface

Herb formula: Yu Ping Feng San

2.2.2. Disharmony of Ying and Wei qi

Clinical manifestations: sweating, aversion wind, general ache, alternate chills and fever, slow pulse, thin fur

Treatment principle: regulating Ying and Wei qi

Herb formula: Gui Zhi Tang

2.2.3. Yin deficiency

Clinical manifestations: night sweating, hot sensation in five middles, hot flush, red tongue with thin yellow fur, thin and rapid pulse 

Treatment principle: tonifying yin

Herb formula: Dang Gui Liu Huang Tang

2.2.4. Pathogenic heat

Clinical manifestations: persistent perspiration with sticky or yellow-stained sweat, red complexion, fever, yellow fur, rapid and big pulse 

Treatment principle: clearing up heat and resolving dampness 

Herb formula:  Long Dan Xie Gan Tang

3. Blood Syndrome (Xue Syndrome)

It is a hemorrhagic disease including hemoptysis, hematemesis, hemafecia,  hematuria.

3.1 Etiologies and pathogenesis

Pathogenic factors invading

Constitution deficiency

Chronic or fabric diseases

3.2 Differentiations and Treatments

3.2.1. Nose bleeding

(1) Attack of the lung by wind-heat

Clinical manifestations: nose bleeding, inching and dry of nose and throat, or fever and other wind-heat symptoms

Treatment principle: dispersing heat, moistening the lung to check the bleeding

Herb formula: Sang Ju Yin

(2) Attack by liver-fire

Clinical manifestations: nose bleeding, headache, dizziness, irritating, red tongue and taut pulse 

Treatment principle: dispersing liver-heat, cooling the blood to check the bleeding

Herb formula: Long Dan Xie Gan Tang 

(3) Qi-blood deficiency

Clinical manifestations: nose bleeding, accompanied with other bleeding, fatigue and pale complexion, and other qi-blood symptoms

Treatment principle: tonifying qi and blood to check the bleeding

Herb formula: Gui Pi Tang

3.3.2. Gem bleeding

Stomach heat (excess or deficiency)

Clinical manifestations: gem bleeding, bright red blood, red tongue with yellow fur, rapid pulse; or often bleeding, the key broken, red tongue, thin and rapid pulse 

Treatment principle: dispersing stomach heat to check the bleeding

Herb formula: Yu Nu Jian

3.3.3. Hemoptysis

(1) Attack of the lung by wind-heat

Clinical manifestations: cough with blood-tinged sputum, inching and dry of nose and throat, or fever, red tongue with yellow fur, rapid pulse

Treatment principle: dispersing heat, moistening the lung to check the bleeding

Herb formula: Sang Xing Tang

(2) Attack by liver-fire

Clinical manifestations: cough, blood-tinged sputum or blood sputum,  hypochondria full pain, irritating, red tongue and taut pulse 

Treatment principle: dispersing liver-heat, cooling the blood to check the bleeding

Herb formula: Xie Bai San added Geode San 

(3) Yin deficiency

Clinical manifestations: dry cough with little amount sputum tinged by blood, and other yin deficiency symptoms

Treatment principle: tonifying yin and  moistening the lung to check the bleeding

Herb formula: Bai He Gu Jin Tang

3.3.4. Hematemesis

(1) Accumulation of heat in the stomach

Clinical manifestations: distress and distention in the epigastrium and abdomen, vomiting blood and food, foul breath, black stool, red tongue with yellow fur, rapid pulse 

Treatment principle: dispersing heat from stomach to check the bleeding

Herb formula: Xie Xin Tang added Shi Hui Tang

(2) Qi deficiency

Clinical manifestations: chronic vomiting blood, the amount often not too big,  distress and distention in the epigastrium and abdomen, black stool, pale complexion, pale tongue with white fur, weak, deep and thin pulse 

Treatment principle: tonifying qi to check the bleeding

Herb formula: Gui Pi Tang

3.3.5. Hemafecia

(1) Accumulation of heat-damp in intestine

Clinical manifestations: stool and bleeding with bright color,  abdomen pain, foul breath,  red tongue with yellow fur, rapid pulse 

Treatment principle: dispersing heat from xiajiao to check the bleeding

Herb formula: Di Yu Wan or Huai Jiao Wan

(2) Spleen and stomach yang-qi deficiency

Clinical manifestations: stool and bleeding with dark color,  abdomen pain, pale complexion, pale tongue with white fur, thin, deep and weak pulse 

Treatment principle: tonifying spleen to check the bleeding

Herb formula: Huang Tu Tang

3.3.6. Hematuria

(1) Damp-heat in the lower jiao

Clinical manifestations: urine with bleeding and bright color, often painful and urgent urination, low abdomen pain, red tongue with yellow fur, rapid pulse 

Treatment principle: dispersing heat from lower jiao to check the bleeding

Herb formula: Xiao Ji Yin Zi

(2) Spleen and kidney deficiency

Clinical manifestations: urine with bleeding and light color, often accompanied with other bleeding, pale complexion, pale tongue with white fur, thin, deep and weak pulse 

Treatment principle: Tonifying qi to check the bleeding

Herb formula: Gui Pi Tang

4. Fever due to internal disorders

It is fever caused by deficiency qi and blood or imbalance of yin-yang.

4.1 Etiologies and pathogenesis

Emotional changes

Blood stasis

Middle jiao qi deficiency

Blood deficiency

Yin deficiency

4.2 Differentiations and treatments

4.2.1 Liver stagnate fire 

Clinical manifestations: low fever or only the hot sensation, which directly related emotions, wiry pulse  

Treatment principle: soothing liver and clearing heat

Herb formula: Dan Zhi Xiao Yao San

4.2.2 Blood stasis

Clinical manifestations: fever in afternoon or night often with fixed and shape pain in the body or masses, purple tongue

Treatment principle: promoting blood circulation to remove blood stasis

Herb formula: Xu Fu Zhu Yu Tang

4.2.3 Qi deficiency

Clinical manifestations: Low fever worse after exertions, pale face, tired, short breath, pale tongue, and weak pulse 

Treatment principle: tonifying qi

Herb formula: Bu Zhong Yi Qi Tang

4.2.4 Blood deficiency

Clinical manifestations: Low fever, pale on the face, nails, lips, often dizziness, palpitation, pale tongue, weak pulse

Treatment: tonifying blood and qi

Herb formula: Gui Pi Tang

4.2.5 Yin deficiency

Clinical manifestations: chronic low fever in the afternoon or night, five middles hot sensation, night sweating, emaciation, tired, red tongue with yellow dry coating, rapid and thin pulse   

Treatment principle: nourishing yin and clearing fire

Herb formula: Qing Gu San

5. Xiao-Ke syndrome (TCM diabetics)

It is disease marked as polydipsia, polyphagia, polyuria, emaciation and sweet urine.

5.1 Etiologies and pathogenesis 

Overeating greasy and sweat food

Emotion disorders

Overworking and aging

5.2 Differentiations and treatments

5.2.1 Upper-xiao Diabetes

Clinical manifestations: thirst and polydipsia, as well as polyphagia, polyuria, emaciation and sweet urine

Treatment principle: eliminating the retained heat in upper jiao   

Herb formula: Xiao Ke Fang 

5.2.2 Middle-xiao Diabetes

Clinical manifestations: hungry and polyphagia, as well as polydipsia,  polyuria, emaciation and sweet urine

Treatment principle: eliminating the retained fire in the middle jiao   

Herb formula: Yu Nu Jin

5.2.3 Lower-xiao Diabetes

Clinical manifestations: thirst and polydipsia, as well as polyphagia, polyuria, emaciation and sweet urine

Treatment principle: tonifying kidney yin  and eliminate the heat   

Herb formula: Liu Wei Di Huang Wan
