Chinese Medicine Lung Disorders 

In Chinese Medicine, Lung is in the chest and the main functions are controlling the superficial (Biao) and reparatory (Qi), as well as distributing the water.  Therefore the main causes of lung disorders are External Pathological Factors (EPF) invading and the main disorder is the Qi reversed, such as cough and asthma.  

1. Gan Mao (The Common Cold)

Gan Mao is a disorders due to EPF invading the superficial and mainly manifested as fever, chill, body pain, nose congestion, sneezing and cough, tec.

1.2 Differentiations and treatments

1.2.1 The Common cold due to wind-cold

Clinical manifestations: aversion cold, slight fever, headache, no sweating, clear discharge, sneezing, coughing, thin and white tongue fur, superficial or / and tense pulse

Treatment principle: To relieve exterior syndrome with pungent and warm herb

Herb formula: Jing Fang Bai Du San

1.2.2. The Common cold due to wind-heat

Clinical manifestations: slight aversion cold, fever, headache, sweating, yellow discharge, sneezing, coughing, thin and white or yellow tongue fur, superficial or / quick pulse

Treatment principle: To relieve exterior syndrome with pungent and cold herb

Herb formula: Yin Qiao San

1.2.3. The Common cold due to heat-damp

Clinical manifestations: slight aversion wind, fever, headache, slight sweating, sticky discharge, sneezing, coughing, heavy feelings in the body, yellow and greasy tongue fur, superficial and quick pulse

Treatment principle: To relieve exterior syndrome with cold and aromatic herb

Herb formula: Xian Jia Xiang Ru Yin

1.2.4 Due to qi deficiency

Clinical manifestations: aversion cold, fever, headache, no sweating, clear discharge, sneezing, coughing, lassitude, thin and white tongue fur, superficial and weak pulse

Treatment principle: To relieve exterior syndrome and tonifying qi

Herb formula: Shen Su Yin

1.2.5 Due to yin deficiency

Clinical manifestations: slight aversion cold, fever, headache, night sweating coughing, hot sensation, red tongue, thin and weak pulse

Treatment principle: To relieve exterior syndrome and tonifying yin

Herb formula: Jia Jian Wei Rui Tang

2 Ke Sou (Cough)

Cough is caused the lung qi reversed.

2.1 Etiologies and pathogenesis

2.2 Differentiations and treatments

2.2.1 Cough due to invasion of pathogenic factors

Cough often is new, acute one, accompanied with exterior syndrome.

(1) Invasion of wind-cold EPF

Clinical manifestations: cough, thin and white sputum, aversion cold, slight fever, headache, no sweating, clear discharge, sneezing, thin and white tongue fur, superficial or / and tense pulse

Treatment principle: relieving wind- cold exterior syndrome and soothing lung qi to stop cough

Herb formula: Ma Huang Tang

(2) Invasion of wind-heat EPF

Clinical manifestations: cough with think or yellow and thick sputum which is difficult to expectorate, turbid nasal discharge, fever, headache, sweating, red tongue with yellow fur, rapid pulse

Treatment principle: relieving wind-heat exterior syndrome and soothing lung qi to stop cough

Herb formula: Sang Jiu Yin

(3) Invasion of wind-dry EPF

Clinical manifestations: dry cough or with scanty and sticky sputum, dryness of the nose and throat, or accompanied with slight aversion wind, fever, white or yellow tongue fur, superficial and quick pulse

Treatment principle: dispersing wind in the lung, moistening dryness to stop cough

Herb formula: Sang Xing Tang (Wind-heat-dry)

                                 Xing Su San (Wind-cold-dry)

2.2.2 Cough due to internal disorders

Cough often is chronic or repeated one.

(1) Phlegm-dampness stagnation in lung

Clinical manifestations: cough with profuse thin and white sputum, white or yellow greasy tongue fur, and slippery pulse

Treatment principle: drying dampness and clearing phlegm to stop cough

Herb formula: Er Chen and San Zi Yang Qin Tang (Cold phlegm)

                       Qing Jin Hua Tan Tang (Hot phlegm)

(2) Invasion of the lung by liver fire

Clinical manifestations: Dry- spasm cough, or with scanty and blood-tinged sputum, hypochondria pain, irritability, red face and eyes, red tongue with yellow fur, taut and rapid pulse

Treatment principle: dispersing heat in the liver, clearing fire, moistening the lung to stop cough

Herb formula: Xie Bai San and Dai Ge San

(3) Lung yin deficiency

Clinical manifestations: Dry cough, or with scanty and blood-tinged sputum, yin deficiency symptoms, red tongue, thin and rapid pulse

Treatment principle: Tonifying yin and dispersing phlegm, moistening the lung to stop cough

Herb formula: Sha Shen Mai Men Dong Tang  

3. Xiao Zheng (Asthma)

It is recurrent attacks of paroxysmal dyspnea with wheezing with high-pitched sound that rattles in the throat; even the patient cannot lie down down.

3.1 Etiologies and pathogenesis

Diets

Constitution deficiency

Any reason causing phlegm stagnated in the lung is basic pathological change, and then pathological factors invading the lung cause asthma.

3.2 Differentiations and treatments

3.2.1 Attack stage

(1) Due to cold invading

Clinical manifestations: paroxysmal dyspnea with wheezing with high-pitched sound that rattles in the throat, even the patient cannot lie down, cough, white phlegm, yellow-blue complexion, white tongue fur, superficial and tense or taut and tense pulse.

Treatment principle: Dispersing cold and warming lung, removing phlegm to stop asthma

Herb formula: She Gan Ma Huang Tang

(2) Due to heat invading

Clinical manifestations: paroxysmal dyspnea with wheezing with high-pitched sound that rattles in the throat, even the patient cannot lie down, cough, yellow and stick phlegm, red complexion, red tongue with yellow-greasy fur, rapid  pulse.

Treatment principle: Dispersing heat and soothing lung, removing phlegm to stop asthma

Herb formula: Ding Chuan Tang

3.3.2 Remission stage

(1) Qi deficiency

Clinical manifestations: lassitude, spontaneously sweating, short breath, poor appetite, pale complexion, white tongue fur, weak pulse.

Treatment principle: Tonify qi

Herb formula: Yu Ping Feng San and Si Jun Zi Tang

(2) Kidney deficiency

Clinical manifestations: short breath, lumbago, weak limbs, pale complexion, white tongue fur, weak and deep pulse.

Treatment principle: Tonify kidney

Herb formula: Liu Wei Di Huang Wan  

4. Chuan Zheng (Dyspnea)

It is a disease marked by the short breathes, orthopnea, breathing with raised shoulders and flapping of ala nasal, cyanosis of lips and a purple tongue, even the patient cannot lei down.

4.1 Etiologies and pathogenesis

Pathogenic factors invading

Emotional changes

Retention of excessive turbid phlegm

Lung and Kidney deficiency

4.2 Differentiations and treatments

4.2.1 Excess type

(1) Lung invaded by cold

Clinical manifestations: short breath, orthopnea, breathing with raised shoulders and flapping of ala nasal, cyanosis of lips and a purple tongue, even the patient can not lay down, cough, white phlegm, yellow-blue complexion, white tongue fur, superficial and tense or taut and tense pulse.

Treatment principle: Dispersing cold and warming lung, removing phlegm to stop asthma

Herb formula: Ma Huang Tang

(2) Lung invaded by wind-heat

Clinical manifestations: short breath, orthopnea, breathing with raised shoulders and flapping of ala nasal, cyanosis of lips and a purple tongue, even the patient can not lay down, fever, yellow and stick phlegm, red complexion, red tongue with yellow fur, rapid pulse.

(3) Turbid phlegm blocking the lung

Clinical manifestations: short breath, orthopnea, breathing with raised shoulders and flapping of ala nasal, cyanosis of lips and a purple tongue, even the patient can not lay down, profuse stick phlegm, greasy fur, slippery  pulse.

Treatment principle: Resolving phlegm and soothing lung

Herb formula: Er Chen pulsed San Zi Yang Qin Tang

(4) Qi stagnation

Clinical manifestations: asthma often caused by emotion changes, short breath, orthopnea, chest full and pain,  white fur, taut pulse.

Treatment principle: Relieving qi stagnation and soothing lung

Herb formula: Wu mo Yin Zi

4.2.2 Deficiency types

(1) Lung deficiency

Clinical manifestations: chromic asthma, lassitude, spontaneously sweating, short breath, poor appetite, pale complexion, white tongue fur, weak pulse.

Treatment principle: Tonify lung qi

Herb formula: Sheng Mai San and Bu Fei Tang

(2) Kidney deficiency

Clinical manifestations: chronic asthma, more expiration than inspiration, short breath, lumbago, weak limbs, pale complexion, white tongue fur, weak and deep pulse.

Treatment principle: Tonify kidney

Herb formula: Jin Gui Shen Qi Wan plus Shen Ge San
